1. Remember You Must Enroll
Every benefits eligible employee MUST ENROLL. If you fail to enroll, for most employees, Prudential's "Low-Option" HMO will be automatically selected for you. This "default" selection is for employee-only (or "single" as shown on the worksheet) medical coverage and for no other benefits. You will not have an opportunity to change the default coverage until next year's open enrollment, unless you experience a change in your family status.2. For Prudential HealthCare Selections
If you select Prudential as your health care provider, you will need to select a primary care physician (PCP) or enter your current PCP's ID number when you enroll. Your PCP's ID number can be found next to your physicians' name(s) in the Prudential HealthCare Physician Directory or at Prudential's physician directory Web site.3. For United HealthCare Selections
If you select United as your health care provider, you do not need to select a PCP. Members can go to any doctor in the United network. Check the United HealthCare provider directory for a complete list of PCPs or visit United's Web site.4. If Waiving Medical Coverage
Employees who waive medical insurance must provide proof of medical coverage. A copy of a medical ID card is considered proof of coverage and must be brought or mailed to Human Resources no later than the date indicated on your Benefits Enrollment Worksheet.If you waive coverage and fail to provide proof of other medical coverage, you will not receive your CoreChoice Dollars for the year.
